Lay Summary
When a person is admitted to hospital, long-term conditions are often recorded in a person’s clinical note. However, this recording is not structured. This means it cannot be used by risk calculators embedded in the hospital’s electronic patient record (EPR) system to provide health-care professionals with information about a person’s risk of important health events occurring. In this study, we will identify different data sources within the hospital EPR system where a person’s long-term conditions are being recorded in a structured way. We will check that these data sources are accurate so they can be used by the risk calculator to calculate a person’s risk. We will check this by comparing them with a person’s general practice records to see whether they match.
